The ability to share patient-client data between providers and programs is essential to providing comprehensive services, community supports, and care across the continuum, including tackling the complex challenges that come with addressing social drivers of health. To enable this data sharing, we need mechanisms to gather, track, and manage individual’s data-sharing consent, and robust interoperability to ensure data has context, so that it is reliable, actionable, and understood.
One conceptual model of proposed consent-to-share issues is outlined in a new white paper titled “Catalyzing Whole-Person Care: Consent-to-Share is the Key – Insights, Next Steps and a Roadmap.” from Stewards of Change Institute (SOCI). The white paper is the culmination of four years of research, convenings, publications and other activities by SOCI focused sharply on consent-to-share as the key to advancing whole-person care.
